Free Sports Physicals (printable information)
The dates have been set for the FREE Sports Physicals at the Shenandoah Medical Center & Sidney Clinic. The dates are going to be:
Thursday, July 20th 8a-12p (Sidney)
Saturday, July 22nd 9a-1p (Shenandoah)
Thursday, July 27th 8a-12p (Sidney)
Friday, August 4th 8a-12p (Sidney)
Saturday, August 12th 9a-1p (Shenandoah)
Shenandoah Medical Center - We will also have immunizations available at that time so any student that needs immunization updates can get them done while they are in the clinic that day! For all students under 18 years of age must have a parent present for all immunizations.
Sidney Clinic - Immunizations are NOT available during the sports physicals. Also, appointments are required for all sports physicals.

We are offering free sports physicals to Iowa Middle School and High School students.
HS Students - Please bring physical form signed by a parent if the parent cannot be present at the physical.
